Dental Tourism Association Membership Agreement
By entering the word YES in the space below, We hereby agree to the following terms and conditions set forth by the Dental Tourism Association (DTA):
Patient Referrals:
We agree to accept patient referrals from the Dental Tourism Association to provide professional dental services at our clinic. We acknowledge and understand that these referrals are based on our clinic's ability to meet the high standards and requirements set by the DTA.
Referral Fee:
We agree to compensate the Dental Tourism Association a referral fee equal to 20% of the total value of the services provided to the referred patients. This fee will be calculated based on the gross amount charged to the patient before any discounts or adjustments.
Payment Terms:
The referral fee is payable within 30 days of the service completion date. Late payments will incur an additional fee of 5% of the outstanding amount for each month past the due date.
Service Quality and Standards:
We commit to maintaining the highest level of professional standards and quality of care for all patients referred by the DTA. This includes adhering to all local and international guidelines, regulations, and best practices in dental care.
Confidentiality:
We agree to keep all patient information received through referrals confidential and use it solely for the purpose of providing dental services. We will comply with all relevant data protection and privacy laws.
Marketing and Communications:
We agree to receive periodic email communications from the Dental Tourism Association or its authorized agents to the email address provided above. These communications may include updates, marketing materials, and other relevant information related to our membership and the DTA's activities.
Membership Term and Termination:
This agreement is valid for an initial term of one year from the date of acceptance and will automatically renew annually unless either party provides a 30-day written notice of termination. The DTA reserves the right to terminate this agreement immediately in case of any breach of terms or misconduct.
Dispute Resolution:
Any disputes arising out of or in connection with this agreement shall be resolved amicably through mutual discussions. If a resolution cannot be reached, the dispute will be referred to mediation or arbitration in accordance with the applicable laws of the jurisdiction.
Acknowledgment and Acceptance:
